CARE, Inc has created this privacy statement in order to demonstrate our firm commitment to privacy.

CARE does not collect personally identifiable information about individual users, except when such individuals
provide information on a voluntary basis, as when completing a form, for example. CARE generally does not
share, rent or sell personally identifiable information about individuals collected on the website to other
individuals, corporations or organizations. In certain cases, CARE may share names and accompanying
demographic information with like-minded agencies with whom we may collaborate.

Various links on our website require users to voluntarily give us contact information (such as names and email
addresses), financial information (account or credit card numbers) and demographic information (street
addresses, zip code, age, etc.). We use contact information from the forms to send the user information about
our agency or to contact the website visitor when necessary or requested. Financial information and related
demographic data that is collected is used solely for the purposes indicated on the respective form where such
information is submitted (such as, for example, to make a donation to CARE or to purchase admittance to a
special event).

CARE reserves the right to perform statistical analyses of aggregate, non-personally-identifying user behavior
and characteristics in order to measure interest in and use of various areas on the website and to use such
collected information in any way.

HOW INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS
TO THIS INFORMATION


The purposes for which the HMIS user or developer is permitted or required by this notice to use or disclose
protected personal information without the individual’s written consent or authorization are administrative,
programmatic and academic research purposes.

If the use or disclosure is prohibited or materially limited by other applicable law, the description of such use or
disclosure must reflect more stringent law.

Other uses and disclosures will be made only with the individual’s written authorization and that the individual
may revoke such authorization.

The individual rights with respect to protected personal information and the individual may exercise these
rights by contacting their housing case manager.

The HMIS user or developer is required by law to maintain the privacy of protected personal information and
to provide individuals with notice of its legal duties and privacy practices with respect to protected personal
information.

The HMIS user or developer is required to abide by the terms of the notice currently in effect.

CARE, Inc reserves the right to change the terms of this policy and to make a new policy provisions effective
for all protected personal information that it maintains.  CARE, Inc. will attempt to provide individuals with a
revised policy at the time of intake and follow-up appointments.

Individuals may complain to the HMIS user or developer if they believe their privacy rights have been violated.  
To file a complaint with the HMIS user or developer you are asked to meet with the user and state the specific
complaint or write it down and mail it to the user at CARE, Inc. P O Box 552, Jesup, GA 31598.  If the
complaint is for the developer, CARE, Inc will forward the complaint to the HMIS developer Pathways Network.

The individual will not be retaliated against for filing a complaint.  For further information please contact
